Lucky Draw Terms & Conditions

1. NUS Graduates from the class of 2009 are invited to participate in the online survey. Graduates who return the completed survey form are eligible for the lucky draw.


2. Each eligible participant will only be allowed to submit one completed survey form. The Office of Quality Management, NUS (OQM) reserves the right to disqualify from the Lucky Draw any submission that is deemed incomplete.


3. Respondents will be awarded one chance to participate in the Lucky Draw when they complete and return the survey form on or before 31 December 2009.


4. The Lucky Draw will be carried out on 29 January 2010 at NUS (OQM). Winners will be notified by email and/or post. All prize winners must produce their Identity Card and other personal details (on request) for identification/verification to claim their prizes.


5. Any unclaimed prizes 30 days after the date of notification shall be deemed forfeited.


6. OQM reserves the right to replace the prizes with items of similar value.


7. OQM¹s decision on all matters relating to the Lucky Draw is final and no correspondence will be entertained.
